Penn researcher calls for expansion of programs to identify potential drug abusers and protect pain patients
PHILADELPHIA -- Individual use of prescription opioids has increased four-fold since the mid-1990s, in part due to increased awareness of pain control for chronic conditions such as low back pain and fibromyalgia and a Joint Commission mandate that hospitals assess patients' pain as a "vital sign" along with their blood pressure and temperature. During the same timeframe, however, the number of people using these drugs recreationally, becoming addicted to them, and dying of overdoses has also shot up. Today, nearly three quarters of all fatal drug overdoses in the United States are due to prescription drugs -- far outnumbering deaths from cocaine and heroin combined, and often outpacing car accidents as the top cause of preventable deaths.
A Perspective piece published online today in the New England Journal of Medicine outlines a plan for an "ideal" prescription-drug monitoring program that would enable doctors, dentists, pharmacists, researchers and law enforcement officials to access real-time data on patients' prescription drug histories. The authors, medical toxicologists Jeanmarie Perrone, MD, an associate professor of Emergency Medicine in the Perelman School of Medicine at the University of Pennsylvania, and Lewis S. Nelson, MD, a professor of Emergency Medicine at the New York University School of Medicine, say that such programs would allow physicians to take better care of patients with legitimate pain issues as well as identify and intervene to help potential drug abusers, and cut the number of opioids in circulation for illegal sale.
"As the number of deaths associated with prescription-drug use surpasses the number of fatalities from motor-vehicle crashes in many states, we can learn from the success of auto-safety innovations that have mitigated mortality despite increased automobile use over the past three decades," the authors write. "We should initiate active safety measures to address the growing rates of illness and death associated with the pharmaceuticalization of the 21st century."
The idea of state-run prescription-drug monitoring programs dates back to federal legislation authored in 1993 -- long before robust internet use and the development of electronic medical records or e-prescribing systems. Today, 42 states have programs, another six have enacted legislation to develop them, and federal agencies including the Centers for Disease Control and Prevention and the Food and Drug Administration have called for broadening the efforts. But clinician awareness about the tools is poor, and some states, including Pennsylvania, restrict physician access, opening the databases only to law enforcement officials.
The authors note that mounting attention regarding abuse potential of painkillers such as oxycodone and hydrocodone has impaired physician-patient relationships in cases of genuine chronic pain issues. For instance, some recommendations suggest obtaining samples from patients for urine drug screens, or asking them to sign so-called "pain contracts" in which they must agree not to sell or give their drugs away.
To avoid these unintended consequences and improve opportunities to identify and help drug abusers, Perrone and Nelson call for a drug-monitoring system to better inform physician prescribing. Among their recommendations: standardization of the type of information submitted to the databases, and a move toward the use of bar-coded prescription paper to more quickly log entries, or a robust e-prescribing system that would eliminate paper and the resulting prescription fraud and "doctor shopping" that contributes to illicit use of these controlled substances. They also suggest that the programs include tracking of drugs ranging from those with the most potential for abuse and addiction (oxycodone, for instance) to codeine cough suppressants and stimulant drugs that may be sold or misused for cognitive enhancement.
The authors cite several benefits to more robust drug-monitoring program, including the potential to provide clinicians with an early warning that a patient may need drug counseling or treatment -- and an opportunity to intervene while the patient is still in the medical setting. In addition, they believe these programs could help identify patients who are receiving multiple legitimate prescriptions from different prescribers and pharmacies and may be at risk of polypharmacy complications. As an added benefit, they note that prescribers could use the databases to monitor use of their own Drug Enforcement Administration number to detect forged or stolen prescriptions.
"Although updating an existing prescription-drug monitoring database to incorporate these 'ideal' goals would require additional support and money, the potential to protect the public health is substantial," Perrone says.
Perrone and Nelson will speak this week at the Harold Rogers Prescription Drug Monitoring Program National Meeting in Washington, D.C., where lawmakers will convene to discuss ways to make existing prescription-drug monitoring programs more user-friendly and compliant with health care privacy laws, and strategies to ensure that the data can be shared between states.

You see a guy walking a pit bull with a studded collar. What's your first thought: tough guy, right? Well, probably. But chances are he's a conscientious, rule-abiding tough guy. So says a study in the journal Anthrozoos. [Vincent Egan and Jason MacKenzie, Does Personality, Delinquency, or Mating Effort Necessarily Dictate a Preference for an Aggressive Dog?]?
Researchers gave personality tests to 235 subjects?from teens to people in their 60s. Those subjects also rated how aggressive they considered different breeds of dogs to be, from Cocker Spaniels to Pit Bulls, and chose which type they'd most like to own.
As the authors expected, the most unfriendly study volunteers, and the youngest, preferred the meanest breeds. But the stereotyping ends there. Pit bull lovers weren't any more likely to have delinquent past behavior?like carrying weapons for fights?than people who preferred friendly Labs.
In fact, pit bull lovers actually scored higher for conscientiousness on the personality test. Which means they may be more rule-oriented, careful and organized?the perfect candidates for dog training classes, the authors say. So next time you see someone walking a fearsome dog, that person?s bark may be worse than his bite.

While the nation sat transfixed on the so-called career-ending gaffes of wildly popular Newark Mayor Cory Booker, West New York Mayor Felix Roque?for whom I served as campaign manager from December 2008 to August 2009? was committing the career-ending ?gaffe? of getting busted by the FBI. This alone would not be news. It is a rite of passage for Hudson County mayors to host an undercover FBI agent armed with an irresistible sack of hard cash in Town Hall during your first week in office. Most mayors wait a few months before clenching their trembling fists over the bribes or make their underlings do the sordid work. A select few never take the money. Former Hoboken mayor Peter Cammarano eagerly stepped into the obvious mousetrap his first week in office, or so the jury decided.

To his credit, Roque has yet to get caught accepting literal moneybags in exchange for favors. Instead, he was arrested for allegedly hacking into the website of an opponent attempting to put into motion a recall vote against him, and allegedly threatening those who established the website. The mayor and his son appeared in court clad in undershirts, and Roque told the FBI that he ?would not have a problem with serving time in jail because he would work out and read while there.?
Seeing your former boss on Politico, in an article linked to on Drudge?or in the New York Times, in an article linked to at Gizmodo? is not what anyone should call a victory. Without a well-oiled campaign machine (of which I was, unfortunately, a part), no one would have bought into this area doctor?s capacity to run a local government. On the other hand, my last words as campaign manager so many years ago were ?you?re going to end up in jail if you keep acting like this,? so the news isn?t exactly unexpected.
Fresh out of college, I helped organize a recall against then-Mayor Sal Vega, a nefarious character who had raised property taxes to make up for lost state money. A key reason for the financial woes? He had challenged the incumbent state senator for his seat and lost, not only spending many months partially ignoring his job as mayor, but leaving the state of West New York?s representation in the New Jersey Legislature hanging in the balance. Vega was a mess and an easy target, the sort of politician who once managed to insult Cuban political prisoners, American war veterans, and the entire children?s population of West New York in one day (he tried to cancel their respective parades, as revenge on his political enemies). It was not difficult to convince the 45,000 residents of West New York that things couldn?t be worse under any alternative, including this random doctor who, having gotten bored with his office job, decided it was time to move into Town Hall.

While there is no doubt that cancer is a contentious issue around the world, a recent research has claimed to have moved a step ahead in the same field. The article in question was published on a website called Real Clear markets: Bigger than Facebook. Now, I'm familiar with most of the leading education websites but this is a new one for me. I also know many of the experts in undergraduate education but I wasn't aware of Richard Tracinski before today. Oh well, if John Hawks thinks he's worth reading then I'll read the article.
Here's what Tracinski has to say ...
Let's put it this way: if you can build a $100 billion company by using the Internet to replace the college yearbook--imagine what you can do if you use the Internet to replace college.
That's what is just beginning to happen. It all became official when the Massachusetts Institute of Technology appointed as its new president the guy who is responsible for MITx, the school's free online education program.
So far nobody has replaced the college textbook, as far as I know [Free/Cheap Textbooks for Students]. (Trust me, I would know if that happened.) That's not a very good beginning.
It's also not very encouraging that he brings up the MIT experiment since that has not proven to be very successful [On the Quality of Online Courses]. It looks like Tracinski has fallen for the standard hype that just because MIT offers free online courses, colleges are doomed.
What makes MITx so interesting is that it isn't just a bunch of lectures posted online. It also includes discussion groups and coursework and a certification program for completion of the work. My first thought when they launched MITx was that it's a little unclear how such a "certificate" differs from a "degree." In turn, that raises questions about how universities are going to be able to keep on jacking up their tuition every year and expecting that students go $100,000 in debt, when so much top-quality education is becoming available for free.
First, there's no evidence that what MIT is offering counts as "quality education." I doubt that Tracinski would know the difference between quality education and a teapot. Second, students who are willing to pay huge tuition fees probably see something of value beyond what they could get by sitting in their bedrooms watching a computer monitor. Maybe the pubs near Harvard Yard really are more exciting than your parent's house in the suburbs of Kansas City?
Thirdly, the discussion about online education goes far beyond the United States of America. If you're going to develop an argument based on capitalist principles then you're forced to ignore all those students in countries where high tuition fees aren't much of an impediment.
The article notes that the new president's main job will be to raise money: "Left unspoken were the unquestionable expectations for Mr. Reif as a powerhouse fund raiser. MIT raised $3 billion over the course of Ms. Hockfield's presidency, and the university is preparing to embark on a new capital campaign." Well, that's one potentially viable new business model: raise billions in donations so that you can use the Internet to offer a top-quality education to a huge number of people for free.
One thing is clear. If you're going to MIT for the sole purpose of taking classes on campus then why would you do that if the school claims to be offering classes of equal quality online?
That's one heck of a "business plan." Give up all tuition income and rely on alumni and businesses to fund free online education!
But there are other business models, too. Newsweek just reported on the start-up of Coursera, a kind of free, online Ivy League college.
"Daphne Koller and Andrew Ng believe the Internet should allow millions of people to receive first-class educations at little or no cost....
"So far they've signed up volunteer professors from Stanford, Princeton, University of Michigan, and University of Pennsylvania. Coursera will offer 35 courses in subjects ranging from math and computer science to world history and contemporary American poetry. These aren't just videotaped lectures; they're full courses, with homework assignments, examinations, and grades"
Let's make one thing perfectly clear. No matter what you might think of online courses in the humanities, mathematics, and computer science. It's not possible to teach most science courses online. The labs are really hard to do when you are a thousand miles away from the test tubes, chemicals, centrifuges, and fossils. You simply can't get a quality education in the sciences without experiencing the lab component of the courses.
Of course, that's of no significance to Robert Tracinski because he probably doesn't think you need to be scientifically literate in order to get an undergraduate degree.
Note that this, too, is so far being run as a charity, though it is the instructors who are donating their time. But Coursera is also backed by Silicon Valley venture capitalists, and the most interesting part of the story is that they're starting to home in on the central economic factor that accounts for the value of higher education: its function as a kind of corporate head-hunter.
"Coursera doesn't pay its professors, and it has yet to dream up a way to generate revenue, though as Ng says, 'If you're changing the lives of millions of people, there will be a way to make money from that at some point.' One possibility involves charging companies for helping them find qualified job applicants"
Consider what a radical advance this would be for young people: not only is your education free, but it includes a job-placement service.
Now I get it. It's bad enough that universities are coming under the influence of large corporations, now the goal is to cut out the middle man and just have the corporations directly fund a company to prepare their future employees.
I wonder if John Hawks realizes that anthropology might not be one of the courses that major corporations want their employees to take? It certainly looks like biochemistry is going to disappear since we can't train biochemists over a high speed modem connection.
It's certainly a much better deal than the old system. I just came across an argument that it's immoral to offer unpaid internships, which actually prepare young people for a career. Yet somehow it's considered perfectly normal to charge someone $100,000 or more for a degree from a college that has deliberately neglected to ensure that its service has any marketable value.
So the purpose of a university is to prepare students for a career but many universities are charging a lot of money and not preparing students for a career? We can fix this by offering free online courses designed and funded by Silicon Valley venture capitalists?perhaps with the help of Ayn Rand and the Tea Party?
If this business plan is successful, it could do wonders for universities in Canada and other countries. Not only would we get an influx of liberal-minded American students, but our own graduates would become increasingly attractive to many American employers who don't buy into this corporate takeover of universities.
I don't know if either of these new business models--MITx or Coursera--is going to become the new model for higher education. Maybe both will be. But a lot of people are now at work on this problem, and they will eventually come up with the right combination to burst the higher education bubble and take down existing universities in the same way that the Internet has taken down the old newspaper publishing business.
I'm beginning to see why people like Robert Tracinski are so appealing to the American right wing. It's because they are stupid.
There's much more but I'm getting nauseous. Let me just close with this ...
One of the radical changes I think we will see is the decoupling of the humanities from technical and professional education. As it is, universities package together two forms of education with radically different economics. Scientific, technological, and professional courses teach skills that are judged by objective standards and have direct, measurable economic value.
The humanities, at best, have an economic value that is indirect and difficult to quantify. Perhaps it will make you more creative and a deeper thinker. Maybe Steve Jobs sitting in on classes in calligraphy helped inspire the Macintosh. But then again, the humanities departments are also packed with a bunch of charlatans who will waste your time with things like--well, here's an example. Check out a hilarious review by Joe Queenan of an impossibly pretentious and utterly nonsensical academic tome on the deeper meaning of that important subject, Harpo Marx.
As someone who came out of the humanities departments--I have a degree in philosophy--I assure you that this sort of thing goes on all the time, and your tuition dollars are paying for it. Obviously, there is no reason why they should pay for it, so eventually they won't.

With its 50-lumen rating, the LCOS-based AAXA P3 ($270 street) isn't the brightest pico projector available. The ?Optoma Pico PK320 Pocket Projector ($450 street, 3 stars), for one, claims 100 lumens. However, the P3 is bright for the price, and it offers any number of additional features that you might not expect at such a low cost, starting with an HDMI port and the ability to read files from a USB memory key. The combination makes it well worth considering.
One complicating factor is the 1,024 by 600 native resolution. This is better resolution than you'll find in most pico projectors, and generally counts as a plus. Both the PK320 and the Editors' Choice Optoma PK301 Pico Pocket Projector ($400 street, 4 stars), for example, offer WVGA (854 by 480). On the negative side, however, 1024 by 600 is unusual enough that most computers don't support it. If you set your computer to another widescreen resolution, like 854 by 480 or 1280 by 800, you'll introduce scaling artifacts caused by adding or dropping pixels to make the image match the projector's resolution.
Among other issues, the artifacts can make text harder to read. The alternative is to stay with standard SVGA (800 by 600), which will avoid the scaling artifacts and give you better image quality than with lower-resolution projectors, but that limits you to a 4:3 aspect ratio. As a practical matter, you may want to stay with 800 by 600 to get crisper data images and reserve the widescreen capability for video.
Basics, Setup, and Connections As with any pico projector, one of the P3's key strengths is its portability. Measuring only 1.4 by 2.6 by 4.6 inches (HWD) and weighing 7.5 ounces, it's small and light enough for a shirt pocket. The power block adds 4 more ounces, but you can also run the P3 from its rechargeable battery, so you don't necessarily have to carry the power block on every trip. You'll probably want to bring it most of the time, however, since the claimed battery life is only 65 minutes.
Setup is mostly standard fare for a pico projector, with an assortment of adaptors to let you connect to a composite video source, a USB memory key (to read common image, audio and movie files), or a computer (using a VGA port). In addition AAXA also sells adaptors (at $12.99 to $19.99 direct each) for iPod, Zune, and PSP connections. The one connector worth special mention, because it's unusual for a pico projector, is a full-size HDMI port, for a computer or video source.
Brightness and Image Quality The P3's 50-lumen rating is admittedly meager compared with most full-size projectors, but it's reasonably bright for a pico projector. As a practical matter, based on the SMPTE recommendations for brightness, 50 lumens is bright enough for a roughly 28- to 37-inch diagonal image at a 16:9 aspect ratio in theater-dark lighting.
One moderate issue is a problem maintaining uniform brightness across the entire screen. On the test unit, I saw vertical strips on both the extreme left and right sides that were noticeably dimmer than the rest of the screen. The issue showed only in screens with light color backgrounds, but is noticeable enough that some people might find it distracting.
Data image quality at 600 by 800 resolution was better than most pico projectors, with the P3 scoring relatively well on our standard suite of DisplayMate tests. Color balance was good, with suitably neutral grays over the entire range from black to white; colors were nicely saturated; and I saw only the slightest pixel jitter with an analog connection on screens that are designed to bring out jitter. Text was easy to read at sizes as small as 9 points.
Video quality was also good for a pico projector, with the P3 doing a reasonably good job in my tests of retaining shadow detail (details based on shading in dark areas) and resisting posterization (shading changing suddenly where it should change gradually.)
Also very much on the plus side is that the P3 shows far fewer rainbow artifacts?with bright areas breaking up into little red-green-blue rainbows?than most pico projectors. Even if you're sensitive to the effect, as I am, it's not likely that you'll see the artifacts often enough with either data or video images to find them annoying.
Other Issues As is standard for pico projectors, the P3 uses LEDs for its light source. More important, the LEDs are meant to last the life of the projector, with a rated 15,000 hour lifetime, so you won't have to spend money on replacement lamps. As is also standard, the 1-watt speakers are hardly worth having. If you need sound, however, the audio out port lets you plug in a headset or external powered speaker.
It would be easier to give the P3 an enthusiastic recommendation if it offered a native resolution that was more widely supported. But even if you ignore the widescreen capability and treat it as astandard SVGA projector, there's still a lot to like. Its balance of small size, low weight, reasonable battery life, and good data and video image quality adds up to a strong argument for the P3. Factor in such niceties as the HDMI port plus the ability to read files from a USB key, and the combination makes the P3 a more than reasonable choice at an attractive price.

I come from a long line of working class folk. High school graduation wasn?t expected until my parents came along, and college was certainly never in the equation for anyone. My grandmothers had worked during the War, but mostly stayed at home. My dad worked a firmly blue-collar union job until his retirement, and my mother worked in telephone customer service until she passed away unexpectedly at 47 years of age. However, my parents and grandparents were influenced by the second wave feminism in the 60?s and 70?s and the messages I received as a young girl all reflected that. I was never told there was anything I couldn?t do. I could go to college, become a lawyer or a doctor or a scientist or the President and get married and have a family and be a shining beacon of success. I would be a modern woman and balance it all.
Then, there I was at 28. MBA, married, career-tracked at a Fortune 100 company, one promotion under my belt, and I was pregnant. I had it all, just like they said I could!
The women I knew who had careers like me and had kids had all resumed their careers. Colleagues kept telling me how bored I would be at home, that they couldn?t wait until their maternity leaves were over because they?d felt so restless. It never even occurred to me that I wouldn?t want to do anything but keep right on working. ?Staying at home? seemed so ? pedestrian. But I knew I wanted to breastfeed, and with my husband in a career that often requires very long hours, we agreed that it made sense to live closer to my job, so I could be the ?primary parent? on workdays. I would do most of the dropping-off and picking-up, but at least I?d only have a five minute commute, whereas his was 45 minutes.
Then my daughter was born, and our reality got a shake-down.
I loved being home with her. We breastfed and co-slept and hung out and played and I was having the time of my life. I wasn?t restless or bored. My daughter got bigger, and as the time of my leave drew to an end, I really hated the idea of going back to work. I started to question why. Why did I want to go back, again? Why was I shopping for someone else to care for my daughter for me? Why was it bothering me so much? What would Gloria Steinem say!? But? this is what modern women do, right? So, I went back, but begrudgingly. I dealt with pumping at work and day care and balanced it all pretty well. Though, I felt like I always left a piece of me at home when I went to work and a piece of me at work when I went home. I continued my success at work and ended up being close to another promotion when I got pregnant with my son a couple years later. I again thoroughly enjoyed my leave, and then went back to work part-time, because maybe that would be better. But my heart wasn?t in it, I was actually tandem nursing at that point, and pumping for my son at work, and co-sleeping, and the incredible hassle of getting two kids to and from day care all wore on me. My husband happened to get a pretty good promotion while I was on leave, and with a little extra help from family to make it happen, I left my job to become a stay at home mom.
The truth of the matter is that the many successful career women ? the ones that hold political office or become CEOs ? aren?t generally women with career husbands and a bunch of kids at home. They stay single, like Janet Napolitano. Or they get married, but don?t have kids, like Angela Merkel. Or they have a kid or two, but their husbands stay home, like Indra Nooyi, CEO of PepsiCo. Or they make enough money to hire a really great nanny and a housekeeper and they either don?t have the hangups and emotional conflict about such a situation that I did (and I certainly don?t judge a woman who can easily compartmentalize such things; in fact I can safely say I am quite jealous of it.) Or they resign themselves to live with those emotions. Tina Fey, in her autobiography Bossypants, describes in detail how she struggles with these very emotions regarding her own family and her thoughts of having a second child. She mentions how she keeps working because she has her dream job and that about 200 people would lose their jobs if she quit working, but that she would love to be at home with her daughter.
My struggle now comes with how I will adapt the messaging I received for my children. Looking back, I know my family meant well, and I love them for embracing the idea that I could be successful and encouraging me to live up to my real potential. I think what their message lacked was any realistic advice about how to make it happen, including the idea of sacrifice and finding balance. Will this shape the college and career advice I give to my daughters? Almost certainly. If they express a real interest in having children, I will encourage them to consider careers that lend themselves better to the flexibility that makes finding that life balance a little easier. If they have incredibly lofty career aspirations, I will make sure they know what personal sacrifices those choices might entail. Additionally, I will tell them that a major consideration in selecting a life partner is one who will truly be a partner in finding that balance. There are women, after all, such as Sheryl Sandberg, COO at Facebook. Her husband is a CEO, and she has also obviously been very successful, and they have kids. But she has said in no uncertain terms that the biggest factor in ?having it all? is who she chose to marry. It will also impact how I talk to my son about what his expectations should be and how he should handle his own roles of husband and father.
Above all, I want them to understand the reality of how difficult these decisions can be, and that they might never figure out that right, perfect balance. Opting out of a career is very scary, but a brave choice for a career woman; we take comfort, create identity, and feel safety in our position, in our success, in our financial security. And while staying home may not be within the feminist messages of our youth, it is an empowering choice for a woman when it is truly a choice and not a prescribed role by her husband or society. And, it turns out, Gloria Steinem agrees.

Google scored an important win over Microsoft a few weeks ago when it won a $35 million U.S. government contract to bring its cloud-based office solution to the Department of the Interior. Microsoft's legacy solutions, of course, remain a staple in government offices, but as more and more agencies want to move their productivity and collaboration services to the cloud, Microsoft is running the risk of losing out in this lucrative market. Today, however, the company is launching a new service that should give more of its government customers, which tend to have very strict data security and privacy regulations, the option to move to the cloud. Microsoft's new Office 365 for Government is, in the company's own words, "a new multi-tenant service that stores US government data in a segregated community cloud."
